How to File a Complaint

The Division of Developmental and Intellectual Disabilities handles complaints regarding agencies that provide services through the Supports for Community Living program or the Community Mental Health Centers.

Complaints may be submitted in writing or by telephone or fax, using the information in the Contact Information box at right, or by e-mail, using the "Contact Us" link in the Contact Information box.
